I am joyce joyce nice to meet you joe jill nice to meet you how are you doing today what brought you to the clinic. My sister joyce she seems to think i have a problem. I i dont know ive just been so tired lately.
I dont have any energy to do anything i am. Im just tired. Im very concerned.
Shes not eating she has lost a lot of weight. She has been skipping a lot of her classes and even her crew team practices. Shes been skipping and she hasnt been on her facebook page and im just very concerned.
Somethings wrong. Jill tell. Me what the typical day looks like for you just a cool practice with the girls i go to classes.
I just studying its i i dont know its its just been too much too much lately. I not do anything ive just been tired i have student loans. I have just paperwork paid just i cant do it anymore.
Im its i need to get some sleep. Im tired. Shes been a straight a student and you know thats why were here i mean.
Shes been like a go getter you know very motivated and you know on top of things and now things are different and we know that somethings wrong you know with her and so thats why were here tell me um how have you handled stressful situations in the past. I dont know was anything in particular that youd like to do when you felt overwhelmed. I like to go for walks.
And run. Listen to r b. Rap.
Music. While im walking i like to go down to the monuments. With with us being here in gw washington area.
It theres so many places to go for walks with so i just thats thats what id like to do thats what i used to do i just dont i cant do anything anymore. Theres nothing and the other thing is last fall. Um.
You know she had told me that this wasnt place. She wanted to be by most of her friends or most of the people in the crew team of sorority snobs and she you know her roommate is never there and most of them drink. And she doesnt drink so.
Thats an added stress right not having friends and then jill brought breaking up with her boyfriend at the beginning another semester. With paul you know thats another thing and then since dad has a new wife and a new baby. Thats been a lot of stress joke.
Have you had any thoughts of harming yourself have you been thinking about death. Ive thought about it its easy i could do it doing cool practice. I can fault lose my balance fall into the water or fall out of the shell and just let go and then just just go ill be gone that thats the best way to do it thats the way ill do it.
Oh my god you had to snap out of this you have to do something i understand. Its a very difficult time for you at this moment. Why dont we work together and create a a plan which will help you to get through this difficult situation.
What we call this is a no suicide contract and basically what this contract includes is you will give me the numbers. The phone numbers of three of your friends that you can call when youre going through some difficult time when some negative thoughts and thoughts of harming himself come to my and which would be the bird. The first person to think about its 100 b.
My sister joyce okay joyce do you mind yeah. I can give you my number okay. And thank.
You thank you very much now the second number. I have a real really a best friend. Shes a real close friend of mine.
Um. When i was in high school. Her name is matilda okay.
I can give you her number. And its three zero one five six seven eight nine six eight. Okay and the third number and my high school counselor for the four years.
I was in high school. Her name is miss wilson and ill give you the number four for her at work. And its three zero one six two nine eight three six five terrific.
Okay so when you feel unhappy. Sad. Depressed.
Oh. Anytime. You have any thoughts of harming.
Yourself. You should call one of this three phone numbers. Either you sister joyce or matilda.
Your high school. Friend or mrs. Wilson.
Your counselor. Besides we can add a few more things in this plan that were just creating. I would like to list a few of your most favorite activities.
So that anytime you feel depressed or sad you just start doing one of those things when its going to be a dance or walk lets lets mention a few here lets write them down. Its the walk in okay and listening to music while i walk or jog. Which is you know my favorite artist is alicia keys.
So alicia. I like her and then just going out to the monuments. Just to see different things in the washington area.
And thats thats those are two things i like to do okay terrific. So this is basically our plan. Im going to give it to you so that you can sign right here at the bottom.

She has become increasingly withdrawn for the past month. And she has a plan for harming herself. And i just completed a no suicide contract with her yeah.
I do have an assessment a my assessment that she is overwhelmed by stress. And does not have much social support here at school.
